Medusa is the encounter that doesn't appear on Romestead's boss list — and the one whose reward is arguably the most interesting item in the game. She's a Desert quest given by a blacksmith, she petrifies you with a stony gaze, and killing her unlocks the Aegis: a shield that turns her own curse back on your enemies. Here's how to start the quest, how to survive her, the turn-in trap that strands people at the finish line, and the full Aegis recipe.

The short version

Talk to a blacksmith first — the quest starts with their warning, not with finding the lair. She's in the Desert. Her gaze curses you, and the in-game hint says protection "may be possible on reflection" — that's a shield. She drops Medusa's Head, which is an ingredient in the Aegis (Blacksmith Level 4). And on the way back, speak to the blacksmith directly, not to the counter.

What we can and can't tell you

The quest structure, the reward chain and the Aegis stats below are all documented and verified. What is not documented anywhere reliable is Medusa's health, armour or resistance values. You'll find specific numbers circulating on other sites; they trace back to a single self-hedged source, so we're not repeating them as fact. We'd rather leave a gap than invent a stat block. Why Medusa isn't on the boss list

This trips people up constantly, and both sides of the argument are half-right.

Romestead's official boss roster has five entries: the Guardian of Minerva, the Cyclops, Pyzifax, the Great Phoenix of Arabia and the Talos Prototype. Medusa is not one of them — which is why our bosses hub lists five and always has.

But she behaves like a boss in every way that matters to a player: a named antagonist, her own lair, a unique mechanic, and a one-of-a-kind reward you can't get anywhere else. So calling her "the sixth boss" is understandable shorthand, even though the game doesn't classify her that way. The practical distinction is that she isn't a progression gate — nothing in the main chain waits on her. She's optional content with an exceptional prize.

Worth knowing for a different reason too: the gorgon appears as a regular enemy in multiple Desert dungeons, not just in her lair. Running into one doesn't mean you've found her.

How to start Medusa's Lair

The most common mistake is going looking for the lair first. You can't meaningfully do that — the quest has to be active before the right dungeon is identifiable.

The trigger

A blacksmith warns you that Medusa is "hiding somewhere in the desert." That warning is the quest start. If Medusa's Lair isn't in your quest log, go and talk to a blacksmith — specifically, the one out in the Desert, since that's who you'll be reporting back to.

This makes Medusa a natural companion objective to the Desert trip you're already making for Tin and Bronze or the Cyclops. You're going to be out there anyway; pick the quest up while you are.

Finding the right lair

Here's the wrinkle that makes this quest confusing: multiple Desert dungeons contain gorgon enemies, but only one of them is the quest location. Clearing gorgon-infested dungeons at random is a good way to waste an afternoon.

The game does solve this for you, though: while the quest is active, the correct lair takes on a unique appearance on your map, distinguishing it from ordinary dungeon markers. So the workflow is:

  1. Get the quest from the blacksmith first. Without it, nothing on the map singles the lair out.
  2. Check your map for the distinctive marker. That's your target.
  3. Ignore other gorgon dungeons unless you want the loot — they aren't her.

The fight: her gaze, and the reflection hint

Medusa's defining threat is exactly what mythology promises: a stony gaze that inflicts a petrification curse. That's the mechanic to plan around, not her damage output.

The game leaves you a deliberate hint about the counter, and it's a pun:

Reflection means a shield. That reading is confirmed by what she drops: the Aegis, a shield whose entire identity is reflecting petrification back at whatever strikes it. The Aegis's own item description makes the theme explicit — "The furious glare of her eyes only open once the shield is struck."

So the practical preparation is:

The turn-in trap

Talk to the blacksmith, not the counter

This is the single most likely thing to strand you, and it happens after you've already won. On returning, you must speak to the desert blacksmith directly — walk up to the citizen and talk to them. Interacting with the building's counter opens the workshop crafting interface instead of the dialogue that completes the quest, so it looks as though the quest is bugged when it isn't.

If your quest log still shows Medusa's Lair incomplete with her head in your inventory, this is almost certainly why. Find the actual blacksmith citizen and interact with them.

The Aegis shield & how to craft it

Killing Medusa unlocks the Aegis recipe at the Blacksmith. It is not automatically granted — you still have to build it, and the requirements are steep.

How good is 50 Block? For context, the starting Wooden Shield blocks 20 across the same 120° arc. The Aegis is two and a half times the block strength for identical coverage — and it converts blocking from a purely defensive act into an offensive one. For a Phalanx build, or anyone tanking in co-op, that's a genuine build-defining item.

Plan the materials before you go

Bronze Bars ×10 need a Level 2 blacksmith just to smelt — but the Aegis itself needs a Level 4 blacksmith, which is the real bottleneck and takes time to grind. Start levelling that citizen early. Amethyst ×5 is a gem with no merchant value, so don't offer your amethysts at the altar for worship if you're planning this — the Aegis is what they're for. Sapphire is the rarest input; budget for hunting one down.

Is it worth doing?

Honestly: it depends entirely on whether you use a shield.

Yes, if you block. The Aegis is the best shield we're aware of in the game and its petrify-on-perfect-block effect is unique — nothing else converts defence into crowd control. For Phalanx players, co-op tanks, or anyone who's found the dodge-timing game unreliable, it's a genuine power spike and worth the Desert detour.

Not urgently, if you don't. Medusa gates nothing. If you're running a two-handed, ranged or thrower build, the reward is a shield you won't equip, and the Level 4 blacksmith requirement plus a Sapphire is a real investment. Do the Cyclops and push toward the Volcanic biome instead, and come back for Medusa when your blacksmith has caught up.

The efficient play either way is to pick up the quest on a Desert trip you're already making, since you'll be there for Tin and the Cyclops regardless.
